The Keto Diet: Therapy or Dietary Fashion? The Uncomfortable Truth About Carbohydrate-Free (or near-carbohydrate-free) Diets

: The ketogenic diet is a diet that generally provides less than 800 kcal/day and less than 50 g/day of carbohydrates, with a protein intake not excessive but sufficient. 80-90% of calories are derived from fat and protein. They have been studied and used for many years in some forms of epilepsy not controlled by drugs, and more recently they are also proposed in the treatment of severe and complicated obesity, when a rapid loss of weight is indicated, for example before a bariatric surgery to reduce the operating risk. The precursors of ketogenic diets were the Very Low Calorie Diet (Vlcd), in which the intake of carbohydrates reached about 100 g/day. But only the most recent ones have a daily amount of carbohydrates between 45 and 58 g, and therefore are sometimes defined as Very Low Calories Keto Diet (Vlckd).
A recent publication in JAMA concludes by saying that: "The widespread enthusiasm for ketogenic diets in treating obesity and diabetes exceeds the evidence available to us. In the field of sport and anti-aging the number of studies is even smaller, so prudence should guide the work of both nutritionists and those who wish to follow them.
